SUMMARY: Ensures the proper delivery of therapy services to residents.

Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities. Performs initial resident examination, evaluation, interpretation of referrals, and creates the initial plan of care for each resident and revises as appropriate. Provides on-site assessment and re-evaluation of each residents treatment per the State or Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate. Coordinates discharge plan and decisions with the PTA
Ensures proper physical therapy care rendered by the PTA for all residents for whom he or she is the therapist of record by delegating appropriate portions of the treatment plan and program to the PTA consistent with the PTA’s education, training and experience; being accessible by telecommunication to the PTA at all times while the PTA is treating resident for whom he or she is the therapist of record; providing supervision of the PTA as required by State Practice Act, and as needed. provides supervisory conferences and visits as required and as requested.
SUPERVISORY RESPONSIBILITIES:
Directly supervises PTA's and carries out supervisory responsibilities in accordance with the facility policies, practices, procedures, and applicable laws. Responsibilities include: interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Indirect supervisor of nurse, program, activity and other assistants as their job functions relate to resident care. Must consistently use discretion and independent judgment.
EDUCATION and/or EXPERIENCE:
High school diploma or general education degree (GED) preferred. Meets state and federal requirements. Long term care and EMR experience strongly preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
Graduate of an accredited education course in physical therapy. Licensed in the state in which they will be practicing. Driver’s license per position requirements.
