Job Description

SUMMARY: Position is responsible to direct, supervise and participate in the day to day operations of the Therapy/Rehabilitation Department.

 ESSENTIAL DUTIES AND RESPONSIBILITIES: An individual must be able to perform each essential duty satisfactorily.   1. Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the dept.


  1. Provide oversight of all activities of the Rehabilitation Department. This includes but is not limited to overseeing the clinical management of the rehab department, including oversight related to case management, quality improvement, care planning, clinical utilization, and patient identification. Assures appropriate rehab staffing and coverage in conjunction with the RRD and recruiting. Assures resident therapy services are provided within the applicable scope of practice. Conducts clinical documentation reviews and financial budgeted oversight and documented education to the units of the team.


  1. Performs administrative duties such as: assigning and overseeing patient/resident care activities, medical record oversight, execution of physician orders, ordering supplies. Manages and adheres to an approved budget. Displays thorough knowledge of the principles, methods, materials, and equipment used in the practice of their discipline and thorough knowledge of all payer source documentation and payment requirements. Provides appropriate consultation to the facility and participates in clinical program development and implementation


  1. Assures effective quality and process improvement activities in the department including attending facility meetings as appropriate, participation in company QA efforts, and other QA activities as needed.


  1. Communicates effectively by collaborating with the Regional Rehab Director in order to meet the facility’s and company’s goals, overseeing interdisciplinary communication regarding resident treatments and discharge planning, and relating professionally, positively, and cooperatively with residents, family members, and other employees at all levels.


  1. Maintains proper documentation. This includes but is not limited to ensuring the submission of required rehab dept reports timely and accurately, assuring that all rehab billing and labor entries are complete, accurate and timely, Assures that company’s documentation standards are met.


  1. Responsible for continuous survey readiness.


  1. Responsible for other duties and responsibilities of annual review and employee evaluation and recognition processes.



  1. Supports and participates in leadership activities and responsibilities. This includes but is not limited to assessing, advising and guiding staff in developing their skills and talents; supporting and enabling employees to use their own   judgment   in   the performance   of their job responsibilities; providing team leadership by building group morale and cohesiveness, managing group process and leading the team toward goal accomplishment; leading all staff in striving to provide the highest quality care and service while continually improving all dimensions of unit and organization performance.


  1. Participates in census building and marketing activities


  1. Presents with professional appearance and attitude at all times. Adheres to facility dress code if applicable. Maintains clean, safe and professional dept. Must be capable of maintaining regular attendance.


  1. Maintains and applies current skills and knowledge through continuing education and in-service programs. Maintain knowledge of Medicare Policies and Regulations, LCDs, and NCDs that directly impact care. Participates in in-service training and completes all mandatory training within designated time frame for self and ensures same for all direct report employees.


  1. Carries out responsibilities in compliance with federal, state, local laws and regulations, and with facility philosophy, policies and procedures.


  1. This position requires standing, sitting, stooping, pushing, pulling, lifting up to 25 pounds or more, bending, climbing, twisting upper body, walking, running, carrying, listening, talking, use of telephone, and exposure to medical equipment and persons with risk of illness.





Directly supervises employees of rehab/therapy department and carries out supervisory responsibilities in accordance with the facility policies, practices, procedures, and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Indirect supervisor of nurse, program, activity and other assistants as their job functions relate to resident care. Must consistently use discretion and independent judgment.


Job Requirements


Graduate of an accredited education course in physical therapy, occupational therapy or speech language pathology or approved education course for physical or occupational therapy assistants.

Prior long term care and EMR experience strongly preferred. Meets state and federal requirements.




Licensed, if applicable, in the state in which they will be practicing. Driver’s license per position requirements.
