Job Description

SUMMARY: Provides quality speech therapy services to residents in accordance with interdisciplinarycare plan and applicable requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES: An individual must be able to perform each essential duty satisfactorily.   1. Proficient administrative and communication within department and facility and company
  2. Thorough knowledge of the principles, methods, materials, and equipment used in the practice of their discipline
  3. Display working knowledge of multidisciplinary practice scopes in the LTC setting
  4. Thorough knowledge of all payer source documentation and payment requirements
  5. Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department.
  6. Performs initial patient examination, evaluation, interpretation of referrals, and creates the initial plan of care for each patient and revises as appropriate. Creates and maintains a patient record for each patient treats.
  7. Provides on-site assessment and re-evaluation of each patient’s treatment according to the State or Federal required interval, whichever is more stringent, and adjusts the treatment plan as appropriate. Coordinates discharge plan.
  8. Partners with multi-disciplinary team to develop plan of care and advocate for patient. Participates in clinical program development and implementation.
  9. Demonstrates effective interdisciplinary communication regarding patient treatments and discharge planning. Cooperates with all facility and corporate departments. Must be able to relate professionally, positively, and cooperatively with patients, patients’ family members, and other employees at all levels.
  10. Demonstrates knowledge and appropriate application of precautions and contraindications to speech language pathology treatment procedures and modalities
  11. Adheres to state, federal and third party billing regulations
  12. Participates in quality and process improvement activities in the department as outlined by the manager/director
  13. Maintains confidentiality regarding patient and company proprietary information
  14. Participates in continuous survey readiness department and facility activities
  15. Promotes and participates in company and facility activities which may include:
  16. Interviews of potential employees
  17. In-service to customers
  18. Marketing and census building
  19. Employee retention activities
  20. Maintain knowledge of Medicare Policies and Regulations, LCDs, and NCDs
  21. Presents with a professional appearance and attitude at all times by adhering to applicable dress codes, maintaining a clean, safe and professional department, promoting positive customer relations.
  22. As directed, assists with survey processes
  23. Maintains proper documentation as directed. This includes, but is not limited to assuring that all rehab billing and labor entries are complete, accurate and timely.
  24. As directed, assists with updating and implementing policies and procedures to reflect the philosophy of the facility, professional standards, and legal requirements.
  25. Maintains and updates current skills and knowledge through continuing education and in-service programs.
  26. Carries out responsibilities in compliance with federal, state, local laws and regulations, and with company philosophy, policies and procedures.
  27. Must speak, understand and read the English language to the extent necessary to safely and properly care for residents.
  28. Excellent interpersonal skills with positive attitude and a high level of energy and enthusiasm. Ability to organize, document, and implement detailed programs. Good verbal and written communication skills.
  29. This position requires standing, sitting, stooping, pushing, pulling, lifting up to 15 pounds or more, bending, climbing, twisting upper body, walking, running, carrying, listening, talking, use of telephone, and exposure to medical equipment and persons with risk of illness.

 

Job Requirements

SUPERVISORY RESPONSIBILITIES:

None - Unless others indicated by position. Example Rehab Manager or Supervising a CFY candidate

EDUCATION and/or EXPERIENCE:High school diploma or general education degree (GED) preferred. Graduate of an accredited education course in speech language pathology. Meets one of the following requirements: The education and experience requirements for a Certificate of Clinical Competence in (speech-language pathology) granted by the American Speech-Language Hearing Association; or Meets the educational requirements for certification and is in the process of accumulating the supervised experience required for certification. Prior EMR/computer experience strongly preferred. Meets state and federal requirements.

CERTIFICATES, LICENSES, REGISTRATIONS:

Driver’s license per position requirements. Licensed Speech Language Pathologist in the state in which they will be practicing.
